Are you a Florida board-approved provider? Yes, the Florida Board of Physical Therapy uses CE Broker to track and verify your compliance. Colibri Healthcare, LLC will report your hours electronically to CE Broker within two business days. Remember, do not forward your CE documentation to the board, keep your certificate in a safe place for your records. What information do I need to provide for course completion and certificate issuance? Please provide your license number on the test sheet to receive course credit. Your state may require additional information such as date of birth and/or last 4 of Social Security number; please provide these, if applicable. Is my information secure? Yes!
